Change of Status Form

When an employer has a change in ownership or business type, trade, or employment in the USVI, the changes are required. All applicable sections of the form must be completed, including the Unemployment Insurance Account Number and FEIN number.

Separations Reporting Form

Quarterly, employers must provide an accurate account of employees, who have separated from the employer. According to Title 29, Chapter 12 subsection 710 c-f, the Virgin Islands Department of Labor (VIDOL) shall monitor resident and non-resident staff that has separated employment by resignation or termination from the beneficiary.

Affiidavit of EDC Beneficiary

The Affidavit is submitted with the quarterly report documents. It affirms and attests to the employment date provided within is true and correct to the best of the ability of the organization submitting the information. No blank lines are permitted, data must be filled out in the statement at the top of the form.

Employer Registration

Employer Registration for Unemployment Insurance

Complete the form in its entirety. Gather your local documents that authorize the business to be lawful in the US Virgin Islands. The registration is processed by the Unemployment Insurance Tax Unit.

Multiple Worksite Report

This Multiple Worksite Report (MWR) collects employment and wages by individual work location in this State. If you operate businesses from more than one location under the Unemployment Insurance Account Number (U.I. Number) shown above, the MWR supplements your Quarterly Contributions Report.

New Hire Reporting

This form is used to report hiring of new personnel.

New Hires should be reported by the 20th day of hire. All sections of the form are mandatory.

AFFIDAVIT OF EDC BENEFICIARY LISTING OF EMPLOYEES

Each quarter, Employers must complete this form and submit the active employees with your organization, it is part of the package that is submitted in paper format and electronically emailed to the Compliance Unit (compliance@dol.vi.gov). This includes employees who are on leave but have not separated from their role. Required on the form is the full name (no nicknames), SSN, Street Address, Position (non-generic job title), Employee Classification, Date of Hire, Date of Severance, Employee Thru, Residency, Date of Resident. The Residency of Beneficiary Instructions can be found within can assist with completion of the form, refer to it as often as needed.
